Additional Test Report Form

A Test Report Form request for electronic scores is free of charge. The first 5 TRF requests for physical copies that are sent via post or courier service are also free of charge (extra fees may be applied for courier services). Any requests made after the 5th copy will have a fee. An administrator will email you separately if you have exceeded your maximum number of free TRFs.

You can request up to 5 physical copies of your TRF. After 5, there is an additional fee and you are required to fill out this form instead:
